Manifest Psychiatry Clinic
New patients:
– Dr. Slider treats kids, teens, and young adults. Caregivers of her patients will be provided with education, parenting tips, and tools to help their loved ones including therapy techniques. She does not do medication management for parents or caregivers. She will assist caregivers in finding their own care if needed.
-Must be a resident of Pennsylvania
– If you are not a resident of Pennsylvania, you will need to be able to travel to PA for your appointment
-Currently out of network with insurance companies. This means that YOU CAN submit to your insurance company for out of network benefits. Click here to learn more about payments and rates.

Patient Passport Portal
Your Elation Passport Portal is linked to the Elation Passport for Patients app. Existing patients use the Elation Passport for Patients app, to communicate with the clinic. You can choose to be notified by text or email for new messages from your doctor. You can also choose to be emailed or texted for appointment reminders.
All communication is directly added to the patient chart.
The Elation Passport for Patients app is used for communication related to:
– invoices/ billing
– booking appointments online,
– rescheduling
– medication refills
– general questions or updates
– upload forms for the clinic or receive forms
-check the date and time of your appointment
– to receive reminders via email or texts for appointments
App is available in Apple Store. Android devices can save this website link to the app on their home screens.
