New Patient Information

Thank you for your interest in becoming a new patient at Manifest Psychiatry PLLC. We hope you will enjoy working with Board Certified Adult, Child and Adolescent Psychiatrist, Dr. Slider. 

New patient appointments are not for emergency or urgent care. We recommend that you continue with your existing psychiatric provider or primary care doctor for urgent care needs.  If you are in need of emergency medical or mental health care please call 911 or go to the nearest hospital emergency department.

New patient appointments are for the following services:

*Full Psychiatric Assessment, Diagnosis, and Evaluation for Children, Teens, and Young Adults

*Second Opinions

*Treatment Planning and Support Team collaboration

*Identify collaboration needs with schools, providers and outside services

*Medication Management

*Parenting Education and Therapeutic Family Support

*School Education, IEP and 504 Recommendations

Insurance & Payment

Our practice is NOT in network with any insurance provider (including UPMC, BCBS, United, Cigna, Aetna, and all  medicaid products). Insurance parameters state that visit lengths and other reimbursement issues should occur in 15-20min blocks.  It is not safe or effective to see patients in 15- 20 minutes blocks. Instead, we take the time to provide parent and patient education, parenting tips, collaboration with schools, and communication with other treatment team members.

Out of Network Reimbursement

Prior to starting care we encourage you to contact your insurance provider by calling the number on the back of your insurance card. Please ask about your out of network benefits. We will provide all billing and insurance required documents to allow you to submit claims for out of network benefits. You can submit this to your insurance company. Your insurance company can then reimburse you for your out of network benefits directly after you submit the claims for services that you pay for by credit card and/or flexible health care spending account. We keep credit cards on file within the medical record system to ensure it is secure. We also utilize a patient portal to allow you to access your medical information at anytime and communicate with Dr. Slider. 


Budgeting

We encourage families to look at insurance deductibles. Many families with high deductible plans may find that the deductible may cover most of the care costs once care is established. 

The average cost for intake is $700 for a 1.5 hr evaluation to $1200.00 for a 3-4hr evaluation. Dr. Slider is willing to split the initial evaluation into two, 90min appointments over 2 months if needed to make it affordable for families.

 The average cost for a follow up appointment is $225- $300 for a 45 min-hour long appointment. A 90 min or more appointment will cost $375.00. These rates were determined by reviewing the average reimbursement rate for insurance companies. New patient evaluations and long appointments are well below insurance rates. 

Dr. Slider also provides care at a discount for multiple siblings and uses a sliding scale rate for families who demonstrate need on a case by case basis. In exchange for lower rates, families have to agree to engage in education or support groups at The Nest, Integrated Services. 

We encourage you to look at your budget to determine if care is affordable.

Please include:

*** Your child’s full name with middle initial

*** Your child’s date of birth (MM/DD/YYYY)

*** Confirmation of your understanding that all services rendered through the practice are not in network with any insurance provider.

*** Parent/ guardian name(s), address(es), phone number(s), and email address(es).

*** Please also send a brief summary that includes your referral source and why your child has been referred for care/ what concerns you have at this time for your child. Please also include the name of the therapist your child is currently working with, if applicable.

Dr Slider will email further instructions if you are appropriate for the clinic.

First Appointment Instructions

First Appointment Information:

Accept your email invitation to set up and download the Elation Passport for Patients app. This app is a patient communication tool that will replace Spruce App ( our phone system) for most communication needs after intake. It directly links to the chart and allows access to information. See clinic page for more details on the Elation Passport for Patients app.

New patient paperwork will be sent through the Elation Passport for Patients app/portal. A computer is required to complete the forms in this app.

 

Update credit card information for Elation via an emailed link. Payments and credit card information is integrated within the medical record. I will send an email to access and update credit card information. You can also find links to update credit card information or pay a bill on the clinic page website

  1. Complete all new intake paperwork/consents and send them back through the Elation Passport for Patient App. The Elation App will add the forms directly into the medical chart. I will send these forms through the Elation Passport for Patients App prior to the first appointment. You must access the patient passport on a desktop for the forms to download.
